FMEA - Failure Mode and Effect Analysis

A method where a team, like a PPDR, (post production design review), team is created to discover possible high risk issues in a design, process, or product and then initiates the most effective strategy to solve the problem by using techniques such as process improvements.

Teams could be formed to monitor external failures.

Can be a step in DMAIC, (define, measure, analyze, improve, control).
